POSITION SUMMARY

To provide superior quality, competitive value, and outstanding service by assuming responsibility for the overall patient experience as it relates to insurance specialty care access. This includes the process of assisting Brevard Health Alliance (BHA) clients with obtaining specialty care as directed by the BHA primary care provider. The BHA Health Service Coordinator works with clients and their families to develop a plan that will help them obtain needed specialty care services.

GENERAL EDUCATION REQUIREMENT

High School Diploma or General Equivalency/Educational Diploma (GED)

ADDITIONAL QUALIFICATIONS

Good communication skills, good analytical skills and well organized Proficient in Information Technology applications Health insurance knowledge preferred Knowledge of medical terminology preferred Customer service experience Effective verbal and written communication skills Ability to manage competing priorities Ability to exercise good judgment Teamwork oriented Resourcefulness in problem solving Able to execute and follow through with delegated tasks and accountability Ability to use or operate computer, copy machine, typewriter, and fax machine Ability to maintain confidentiality at all times and follow Brevard Health Alliance’s confidentiality guidelines Meets department skill requirements

PRIMARY ACCOUNTABILITIES

Maintain ongoing tracking and appropriate documentation on insurance specialty referrals to promote team awareness and ensure patient safety. Per referral guidelines, provide appropriate clinical information to specialist. Contact review organizations and insurance companies to ensure prior approval requirements are met. Present necessary medical information such as history, diagnosis, and prognosis. Review details and expectations about the referral with patients as needed. Refer patients to clinic LCSW as needed. Assume advocate role on the patient’s behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion. Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers. Ensure that referrals are addressed in a timely manner. Ensure that patient’s primary care chart is up to date with information on specialist consults. Follow up with specialists to obtain consult notes if not received. Obtain prior authorizations for all office visits and procedures pertaining to specialist appointment as needed Ensure referrals and authorizations fax inbox is monitored and addressed Remain logged into the phone queue and available for phone calls pertaining to referrals and authorizations Maintain appropriate documentation of all communication to the patient and specialists in patient EMR Assist uninsured patients with referral process to include specialty and sovereign immunity if necessary Perform other related duties as assigned.

PHYSICAL DEMANDS

Visual acuity, hand-eye coordination, hearing ability and dexterity to perform tasks and operate office equipment. Standing, walking, bending, stooping, and lifting as job requires for possibly extensive periods of time. Manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, and other office equipment. Normal range of hearing and eyesight to record, prepare and communicate appropriate reports.

Work in a stressful, public contact environment. Concentration on multiple priority activities in a fast paced, stressful environment. Professionalism, courtesy, and respect required when interacting with patients and staff. Exercising of appropriate judgment when responding to emergency or complex situations. Maintenance of confidentiality in accordance with Health Insurance Portability and Accountability Act (HIPAA). Assess and interpret data to educate staff. Flexibility to work various hours in multiple locations.
