Summary:
Manages and supports the population health team and project staff. Leads, develops, and administers evidence-based population health plan, including performing local needs analysis for program, and service delivery. Develops and is accountable for the measures of success for improving the health of assigned various patient populations (Medicare, Medicaid, commercial, and uninsured) as evidenced by patient activated self-care.
Education:
Essential:
Master’s degree in a clinical/healthcare related field such as Nursing, Pharmacy, Clinical Social Work or Psychology.
Experience:
Seven years of related experience in care coordination, case management or population health.
Credentials:
