Patient Care Coordinator MAT Care Clinics 110 East Broward Boulevard, Fort Lauderdale, FL 33301 The Patient Care Coordinator should exercise independent judgment and is involved in the evaluation of present procedures. Works closely with supervisors and coworkers to complete all aspects of patient scheduling, patient flow, assigned training, obtaining and recording patient health information, supporting providers in non-medical matters, performing tasks /schedule management to help providers run on time, schedule review, Must learn and perform new standards and processes as they are implemented and participate in customer service and patient satisfaction initiatives, and special projects. Must have good organizational, communication and interpersonal skills. Below are the duties and responsibilities for this position: Scheduling patients for office visits and ancillary therapy and diagnostic services Coordinating with GateHouse Treatment and other outside referral sources to manage patient appointment schedules. Checking in patients upon arrival at MCC, providing tours of the office, providing introductions to them clinical staff and obtaining intake consent documentation for first-time patients. Checking out patients following their appointments, scheduling follow-up appointments, tests and ancillary services. Alerting referring parties of late appointments. Maintaining daily Missed Appointments Report and discussing patient appointment issues with referring parties Supporting clinical staff in administrative/non-medical matters. Answering and directing telephone calls, taking messages for providers and staff as necessary. Performing tasks / managing schedules to help keep providers running on time. Creating initial care plans with patients at time of intake and follow-on clinical care plans as patients. Transition to stepped-down and/or outpatient services. Reporting any incidence of

infections to the NH DHHS and referral sources. Assisting the Medical Assistants with diagnostic testing services, including but not limited to COVID tests, weight checks and other basic medical tests. Learning and performing new standards and processes as they are implemented. Participating in patient service/ satisfaction initiatives. Completing all workplace trainings as required. All other duties as needed. HS Diploma or GED preferred. 1 year experience preferred. Proficient in computer skills. Able to follow established procedures. Background Screening, drug screening.
