This role is remote but alsorequires field work 80% of Field Visits (must be able to cover theentire region 7 which includes Orange, Seminole, Osceola andBrevard counties). Will be assessing the Geriatric Population!

Position Purpose:

Assists in developing, assessing, and coordinating holistic caremanagement activities to enable quality, cost-effective healthcareoutcomes. May develop or assist with developing personalizedservice care plans/service plans for long-term care members andeducates members and their families/caregivers on services andbenefits available to meet member needs. Typical Day in the Role8-5 Mon-Fri; Monthly and quarterly member contact and will include80% travel. Remote role. Will require a driver s license.

• Managing a case load for healthcare members with long term careneeds.
• Geriatric long term care
• Member assessments and notes.
• Complete assessments with members, caregivers, or providers toobtain information regarding client status, support system, andneed for services for care plan development.
• Monitor delivery ofservices and follow-up with members, caregivers, or provider sthrough in person visits and telephonic contact.
• Authorize andcoordinate referral for services.
• Ensure provider services aredelivered without gaps and identify functional deficiencies inplans of care.
• Assist in coordinating the development of informalor voluntary services to integrate into the member care planCollaborate with discharge planners, physicians, and other partiesto ensure appropriate discharge plan, care plan, and coordinationof acute care and long-term care services!
• Assist member withfiling and resolving complaints and appeals.

Must haves :

Musthaves:

True case management in the medical field or true fieldexperience 2 years of Care Management experience (field experienceis a must) Home Health Experience Long Term Care Medicaidexperience Medicaid / Medicare experience Need to see experiencebeing able to manage high case load

• Caseloads of 50,60,70 memberswho are 65 years of age and above team is not looking for pediatricexperience Experience with electronic medical health records Niceto haves: 1.

Discharge Planning Disqualifiers:

• Not having fieldexperience
• Not having previous experience with high caseloads
• Not at least meeting 40 WPM on typing test

Licensure Required:

Valid driver’s license
