Region 7 Orlando Clinical – LTSS Service Care Manager Position Available In Broward, Florida

This role is remote but also requires field work 80%of the time in Florida

• Orange County (must be able to cover theentire region 7, see Map), will primarily be supporting Orange andSeminole ( no candidates from East Orlando) Will be assessing theGeriatric Population!

Position Purpose:

Assists in developing, assessing, andcoordinating holistic care management activities to enable quality,cost-effective healthcare outcomes. May develop or assist withdeveloping personalized service care plans/service plans forlong-term care members and educates members and theirfamilies/caregivers on services and benefits available to meetmember needs. Typical Day in the Role 8-5 Mon-Fri; Monthly andquarterly member contact and will include 80% travel. Remote role.

Will require a driver s license.

• Managing a case load forhealthcare members with long term care needs.
• Geriatric long termcare
• Member assessments and notes.
• Complete assessments withmembers, caregivers, or providers to obtain information regardingclient status, support system, and need for services for care plandevelopment.
• Monitor delivery of services and follow-up withmembers, caregivers, or provider s through in person visits andtelephonic contact.
• Authorize and coordinate referral forservices.
• Ensure provider services are delivered without gaps andidentify functional deficiencies in plans of care.
• Assist incoordinating the development of informal or voluntary services tointegrate into the member care plan Collaborate with dischargeplanners, physicians, and other parties to ensure appropriatedischarge plan, care plan, and coordination of acute care andlong-term care services!
• Assist member with filing and resolvingcomplaints and appeals.

Must haves:

2 years of Care Managementexperience (field experience is a must) Home Health Experience LongTerm Care Medicaid experience Medicaid / Medicare experience Needto see experience being able to manage high case load

• Caseloadsof 50,60,70 members who are 65 years of age and above team is notlooking for pediatric experience Experience with electronic medicalhealth records Nice to haves: 1. Discharge Planning Disqualifiers
• Not having field experience
• Not having previous experience withhigh caseloads
• Not at least meeting 40 WPM on typing test.

Candidate Requirements Education/Certification Required:

Requires aBachelor’s degree and 2 4 years of related experience. (BachelorsDegree should be within the realm of Healthcare) Psychology,Sociology, etc. Field experience would need to be long term to havethe team consider someone that does not have a degree within thespace they are looking for.
