Job Description:

Provides case management for assigned patients, utilizingclinical expertise, communication, and problem-solving skills tooptimize outcomes. Performs needs assessments, facilitates referrals, and linkspatients to health, wellness, and post-acute resources across thecare continuum. Promotes interdisciplinary collaboration, care coordination, anddischarge planning to ensure appropriate length of stay, resourceuse, and smooth transitions. Ensures compliance with regulations, documents accurately, andcommunicates with team members, including utilization review nursesand social workers. Facilitates referrals for medications, DME, home health, andhospice; manages barriers and authorization processes. Assists with transfers, advocates for patients, and participates inperformance improvement initiatives. Uses predictive analytics, manages post-acute care needs, andprovides medication support. Requires RN licensure, a bachelor’s degree or equivalentexperience, and 3+ years clinical case management experience.
