The role you’ll contribute:
As part of the HospiceCare leadership team, the Patient Care Manager directs and supervises the day to day patient care delivery by members of the hospice interdisciplinary team. In conjunction with the patient, family and the patient’s attending physician, he/she develops and manages the patient’s plan of care. The Patient Care Manager ensures that care delivery is in accordance with Medicare Hospice Conditions of Participation and within the organization’s budget and productivity guidelines. The Patient Care Manager acts as the clinical resource for staff members, and facilitates weekly interdisciplinary team meetings to discuss the patients’ plans of care and ongoing eligibility for hospice services. The Patient Care Manager is responsible for clinical productivity, education and appropriate documentation by staff to meet the Medicare COPs for Hospice. The Patient Care Manager reports to the Assistant Director of Clinical Services.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES
  • Oversight of clinical team in providing end of life care
  • Supervision of clinical team
  • Orientation and education to clinical staff
  • Audits and chart review
  • EMR training
  • Assessments and guidance to Medicare COPs
  • Data collections and trending of data
  • Hiring and training of clinical staff
  • Manage the entire Home team including non-clinical staff
  • Coordinating schedules for the entire team
  • Directing day to day operations in the department
  • Participates in the hiring process
  • Completes performance evaluations in a timely manner, is involved in coaching, counseling and termination per policies
  • Maintains high level of employee engagement
  • Ability to appropriately handle stress and interact with others
  • Ability to meet Productivity standards
  • Conducts staff meetings with department employees on routine basis
  • Supervises, schedules and assigns duties to all dept staff and coordinates with other dept to provide services timely and professionally
  • Coordinates patient care visits
  • Responsible for patient care satisfaction
    Qualifications

EDUCATION AND EXPERIENCE REQUIRED
  • Registered Nurse
  • 2 years’ experience in critical care nursing
  • 2 years’ experience in Hospice Nursing
  • End of Life Care Issues
EDUCATION AND EXPERIENCE PREFERRED
  • BSN
  • Certification in Hospice and Palliative Care Nursing
  • Customer service skills
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED
  • Registered nurse
  • Active Florida Driver’s license and auto insurance
  • Active American Heart Association BLS
