You will travel weekly to each center to assess, develop and implement performance improvement initiatives, monitor and report on 5-Star Outcomes, Risk Management, and other quality related measures, and will work directly with Administrators, DON’s, MDS and other stakeholders to develop standard operating procedures, action plans and educate staff. approach in order to create change in facility performance and outcomes. Participate in the training & education of facility MDS and Interdisciplinary Team Members in all areas of the Resident Assessment Instrument process, and the quality measures. Ensure that facilities are accurately reporting resident conditions and services provided in accordance with Federal and State guidelines in an effort to maximize quality measure ratings, and reimbursement through all payer sources. Actively participate in the quality improvement process for the facility Participate in and/or provide in-service education sessions Schedule, direct and document regular meetings with nursing staff to assure effective communication

Requirements:

Associate’s Degree in Nursing (ASN) or above required. Bachelor’s or Master’s Degree in Nursing a plus. Compact RN Licensure, or RN Licensure in one of the following states: Florida, Georgia, or South Carolina and is able to apply for a compact Licensure. 5+ years nurse management experience in a long-term care or skilled nursing center required 2+ years experience as a Director of Nursing, Clinical Director and/or Regional Nurse Consultant in SNF/LTC setting required. Prior LTC or SNF experience in the state of Georgia, Florida, or South Carolina required. Regional experience strongly preferred. Multi-site clinical leadership experience required Prior Clinical Quality, Performance and Process Improvement, Regulatory and Compliance, and Risk Management experience required. Must have experience with Internet-based Applications, EMR Software (Matrix Care) and Windows and Microsoft Office programs.
