Mileage Reimbursement Key Responsibilities:
- Communicate successfully with peers, supervisors, public, and staff members in a manner that represents Hope’s culture of a collaborative, positive work environment.
- Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff on a daily basis.
- Involved in the development and implementation of quality assessment and performance improvement (QAPI) activities.
- Provide regular updates to team on participants’ status in therapy.
- Coordinate services according to the plan of care with external vendors, ensuring appropriateness of treatment plans and interventions.
- Assess the need for adaptive equipment and durable medical equipment (DME); order and facilitate authorization of such equipment; assess, recommend, and provide follow-through therapy for specialized shoes.
- Participate in meetings as indicated (morning, team, family, nursing facility, assisted living facility, and hospital).
- Provide treatment as indicated.
- Utilize appropriate safety measures when providing care.
- Develop special programs according to participant needs.
- Educate staff, participants, and family on services available through therapy staff.
- Develop, instruct, and supervise other staff, and/or family members on therapy program to be carried out by individuals other than a physical therapist.
- Provide orientation and training in regards to body mechanics and lift equipment to staff as requested.
- Communicate observed staffing needs/changes and further program development needs to supervisor.
- Complete documentation as required.
- Adjust schedule as needed to meet the needs of PACE program participants.
- Maintain confidentiality regarding participant, staff, contractor, and organizational information.
Knowledge & Skills:
- Excellent communication; via phone, email, and in person.
- Attention to detail & ability to handle confidential information following HIPPA guidelines.
- Must be flexible to work in other locations if needed.
Required qualifications: - Degree in Physical or Occupational Therapy required.
- Licensure required.
- One year elder care work experience with the frail elderly required.
- Management experience preferred.
- Valid Florida Drivers’ License with good record

- Clear level 2 background check – Active BLS for healthcare professionals from the American Heart Association or Red Cross; Current employees must obtain by January 1, 2025.
