Full-Time Position Summary:
We are seeking an experienced and compassionate Clinical Manager with a strong background in home health to join our leadership team. This individual will play a pivotal role in overseeing clinical operations, ensuring the highest quality of patient care, and maintaining regulatory compliance within our home health agency. The Clinical Manager will serve as a mentor and leader to the clinical staff, driving performance, patient outcomes, and operational excellence.
Key Responsibilities:
Supervise and support clinical staff including RNs, LPNs, therapists, aides, and other home health professionals Ensure compliance with state, federal, and accrediting body regulations (e.g., CMS, Joint Commission) Review and approve patient assessments, care plans, and documentation Conduct routine case conferences and participate in interdisciplinary team meetings Monitor clinical performance, caseloads, and patient outcomes Provide field supervision and performance evaluations as needed Collaborate with the intake, scheduling, and administrative teams to ensure efficient operations Assist in hiring, onboarding, and training new clinical staff Participate in quality improvement initiatives and ensure documentation accuracy Serve as a liaison between clinical staff, physicians, patients, and families Registered Nurse (RN) license in good standing, required Minimum 2-3 years of home health experience , with at least 1 year in a leadership or supervisory role Strong knowledge of OASIS documentation and home health regulations Excellent organizational, interpersonal, and communication skills Ability to lead by example and support a positive team culture Proficient in electronic medical records (EMR) systems – prior experience with [insert system, e.g., Kinnser, Homecare Homebase] preferred CPR Certification and valid driver’s license
