Job Title:

Patient Coordinator Job Summary Perform clerical duties utilizing specific knowledge of medical terminology, PACE regulations, contract providers and authorization procedures. Duties include scheduling appointments for participants, including coordination with caregivers, ALF’s, escorts and transportation. Pull reports from information system to monitor compliance. Oversee scanning of assessments by medical receptionist into software system. Essential Job Functions Schedules all participant diagnostic appointments and medical consultation for PACE participants and enters appointments into PACE medical authorization system. Tracks appointments and produces reports as requested in order to track data. Coordinates with caregivers, transportation department, ALF staff and escort, if needed, in order to assure that participant attends the appointment. Oversees unit specific documentation as per department guidelines and regulatory standards. Provides necessary coordination with other departments Acts as support for receptionist Performs other support functions as needed. Attends department meetings and in-services as needed.

Minimum Qualifications:
Education:

High School Diploma or equivalent

Experience:

Two years office/clerical experience preferably in healthcare setting

Licenses/Certifications:

N/A Abilities Required Bilingual (Spanish/English) preferred Knowledge of medical terminology. Ability to work with geriatric participants Ability to gather data, compile information, and prepare reports Must be able to speak, read and write English.
