$120k•$155k (based on years of experience) + 15% annual target bonus

Key Responsibilities:

Lead and manage all aspects of daily case management operations Oversee core case management functions, including discharge planning, care coordination, and utilization review Ensure staff compliance with internal policies and external regulatory requirements Manage staffing resources and department budget to support productivity and quality targets Serve as a change leader and coach, fostering a culture of accountability and excellence Uphold the organization’s Code of Conduct and core mission/values

Requirements:

Bachelor’s degree in Nursing (required); Master’s degree in Nursing, Health Administration, or Business Administration (preferred) Current RN license (Required; LCSW not accepted) Minimum 2 years of recent acute care case management experience Minimum 5-7 years of total clinical experience Strong leadership, team-building, and communication skills Ability to drive change and manage complex operational initiatives across departments
