Full Job Description Job Summary:

Surgical office seeking dependable, adaptable Patient Care Coordinator to ensure accurate patient charts, coordinate referrals, and to provide patients with excellent care. This role will require 2 days weekly of direct patient contact and 3 days for administrative tasks.

Responsibilities:

1. Ensure complete and accurate registration, including patient demographic and current insurance information. 2. Perform all actions to assemble and complete patient charts for appointments with required medical reports, accurate medical history, active medications, and active diagnoses. 3. Triage patients and prepare exam rooms for appointments. 4. Initiate and follow up on patient referrals for physician ordered testing. Review details about the referral with patients. 5. Contact insurance companies where needed to ensure prior approval requirements are met. Provide medical information such as history, diagnoses and CPT codes. 6. Maintain ongoing, timely tracking and appropriate documentation on referrals. 7. Ordering medical supplies and stocking exam rooms. 8. Completion of medical records requests 9. Assist patients in resolving potential issues related to their healthcare, referrals, appointments, etc.
