Referral Coordinator Clearwater Free Clinic 1218 Court Street, Clearwater, FL 33756 Overview The Referral Coordinator is responsible for facilitating timely and appropriate specialty referrals for patients seen at the Clearwater Free Clinic. This position ensures coordination of care with outside organizations, specialists, and charitable health programs while advocating for the patient’s access to necessary services without the use of traditional health insurance. Duties Coordinate specialty referrals for patients in accordance with clinic procedures and eligibility criteria. Establish and maintain collaborative relationships with local charitable care providers, hospital systems, community resources, and specialty offices that participate in low- or no-cost care. Communicate with patients regarding the status of their referrals, preparation requirements, and follow-up instructions. Accurately document all referral activities and outcomes in the clinic’s electronic or paper-based medical records system. Track and monitor the completion of referrals to ensure continuity of care. Educate patients about available resources and assist with program-specific applications as needed (e.g., sliding scale programs, charity care). Ensure referrals are made based on medical necessity, provider recommendation, and eligibility for outside services. Respect patient confidentiality and comply with HIPAA and clinic-specific privacy protocols. Assist with administrative support tasks related to clinical operations as directed by the Clinic Manager or Clinical Director. This position is Monday-Thursday, 8am to 6pm. Qualifications Licensed Practical Nurse (LPN) Minimum of two years of experience as a referral coordinator in an Outpatient clinic. Experience in working with underserved or uninsured populations strongly preferred. Excellent organizational, interpersonal, and communication skills. Ability to work with diverse community partners and navigate charitable healthcare systems. Compassionate, patient-focused approach with strong attention to detail. Proficient in computer systems and medical records documentation.
