JOB SUMMARY
The Lead Case Manager oversees the work of Case Managers and other relevant staff. Moreover, the Lead Case Manager will lead and perform a variety of routine duties in the assigned clinic to assist medical staff in the examination and treatment of patients.
CORE JOB FUNCTIONS
Maintains inventory of medication and places orders in a timely fashion to maintain supply. Ensures policies and procedures are followed by staff and recommends disciplinary action to Site Manager. Works with front desk to ensure all documentation is completed timely and encounters are captured and sent over with daily log. Obtains vital signs and patient information for inclusion in the patient’s medical record. Gives injections or treatments, performs routine laboratory tests, and assists in other patient care activities. Prepares treatment room for patient examination and assists with physical examinations by preparing supplies and handing instruments to the Nurse Practitioner. Provides patients with basic instruction and information regarding examination procedures. Cleans and sterilizes medical instruments, observing the principles of sterile technique. Schedules patients for tests and completes required forms for laboratory work. Maintains accurate and complete patient records and documentation. Orders, stocks, and inventories supplies and assists in performing clerical duties. Escorts and transports patients to various clinic locations. Establishes and continuously assesses the effectiveness of the internal controls within the clinic and compliance with HealthyMD policies and procedures. Ensures employees are trained on controls within the function and on HealthyMD policy and procedures. Tracks clinical certifications for the clinical staff (e.g. 500/501, Oraquick, Phlebotomy, etc.) Leads and Participates in outreach events. Acts as a liaison between DOH and Clinical Staff. Travel to different site location to conduct STD/HIV Screenings to help identify individuals at risk. These events can occur before, during or after normal working hours including weekends or holidays. CORE QUALIFICATIONS
Education:
High school diploma or equivalent Bachelors degree preferred
Certification and Licensing:
Phlebotomy Certification preferred. Certified Medical Assistant Certification preferred
Experience:
Minimum 5 years of relevant experience
Knowledge, Skills and Attitudes:
Knowledge of business and management principles. Ability to direct, manage, implement, and evaluate department operations. Ability to establish department goals, and objectives that support the strategic plan. Ability to effectively plan, delegate and/or supervise the work of others.
