The Clinical Manager:

Demonstrate professional conduct and ethics according to agency policy and procedure.
Maintain confidentiality of patient information and all agency operations.
Provide nursing services in compliance with agency policies and procedures and federal, state and local regulations.
Provide oversight of all patient care services and personnel. The oversight provided must include the following: making or assisting the scheduler with patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed and assuring the development, implementation and updates of the individualized plan of care are including all physicians involved in the plan of care and integrating the orders.
Perform initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
Assist the physician in evaluating the patient’s level of function.
Assist the physician in writing an outcome-based plan of care in coordination with the patient and/or family.
Supervise the nursing clinical staff engaged in direct delivery of home care services in the initial evaluation of patient’s needs, the written goal-oriented plan of care and the implementation of the physician’s plan of treatment.
Coordinate the total plan of care with appropriate resources, conducting periodic reassessments of patient needs and revising the plan of care as needed.
Maintain ongoing communication with the staff and participate in case conferences.
Ensure care provided is based on a continual assessment of the patient that addresses physical, psychosocial, environmental and safety factors, physician’s orders, relevant medical history, functional status and availability of caregiver.
Supervise the implementation of the physician’s plan of care, keep the physician informed of change in condition and obtain physician orders for change in the plan of care as indicated.
Ensure clinical competency of staff by utilizing appropriate teaching methods, including demonstration, verbal instruction and written material.
Ensure care provided is compliant with all agency policy and procedures.
Ensure communication with patient, patient’s family, physician and other members of the healthcare team takes place as necessary in accordance with the agency policy and procedure to provide continuity of care and maintain high quality of all services.
Coordinate services given by other members of the health care team.
Make referrals to other medical services, as needed.
Evaluate the effectiveness and outcomes of care.

Qualifications:

1. Degree in Nursing preferred. Current license as a Registered Nurse in the respective state: ARNP or other advance practice nurse
2. Must have one of the following qualifications: Licensed physician, P.T., SLP, O.T., Audiologist, MSW, RN
3. One (1) year home health care experience preferred with knowledge of state and federal regulations relative to home health. Empath Health values diversity as it strengthens our community and care.
