Job Description

Director of Case Management AceTek Solutions Oviedo, FL 32765 Overview We are seeking a dedicated and experienced Director of Case Management to lead our case management team in providing exceptional care and support to our patients. This role is crucial in ensuring that patients receive comprehensive care plans tailored to their individual needs, particularly in a fast-paced healthcare environment such as a Level I Trauma Center. The ideal candidate will possess a strong background in emergency medicine, mental health counseling, and acute care management, along with a deep understanding of medical terminology and physiology. Responsibilities Develop, implement, and oversee care plans for patients, ensuring they meet the highest standards of quality and compliance. Collaborate with multidisciplinary teams to coordinate patient care from admission through discharge. Conduct assessments to identify patient needs and facilitate appropriate interventions. Manage discharge planning processes to ensure smooth transitions for patients moving from acute care settings. Maintain accurate medical records and documentation in accordance with regulatory requirements. Provide leadership and guidance to case management staff, fostering a culture of continuous improvement and professional development. Stay informed about best practices in behavioral health and emergency medicine to enhance patient outcomes. Engage with community resources to support patients’ post-discharge needs. Requirements Bachelor’s degree in Nursing, Social Work, or related field; Master’s degree preferred. Proven experience in case management within a healthcare setting, particularly in emergency medicine or acute care environments. Strong knowledge of medical terminology, physiology, and behavioral health practices. Familiarity with discharge planning processes and care coordination strategies. Excellent communication skills with the ability to work collaboratively across various departments. Ability to analyze complex situations and make informed decisions that prioritize patient welfare. Certification in case management (CCM) or equivalent is highly desirable.
