Director of Case Management – Oviedo, FL Job Description The Director of Case Management is responsible for overseeing and coordinating all departmental activities to ensure optimal patient care and efficient discharge planning processes. This role requires an executive presence with proven leadership capabilities and the ability to build trust, manage change, and effectively engage interdisciplinary teams and external stakeholders. Responsibilities Direct and evaluate departmental operations, including the case management model, staffing, use of information technologies, onboarding, and staff competencies. Allocate resources effectively to staff the department and meet productivity and quality goals. Oversee core case management functions and practices. Ensure adherence to care coordination and discharge planning processes, maintaining timeliness, quality, and proper documentation. Collaborate with the Facility CFO and Division CM Leader to achieve established goals and expectations. Assess and improve departmental performance by evaluating operational processes, monitoring performance data, and implementing sustainable performance improvement activities. Ensure compliance with policies and standard operating procedures. Establish working relationships with key stakeholders, including CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal. Perform other duties as assigned. Adhere to the ‘Code of Conduct’ and ‘Mission and Value Statement’. Essential Skills Minimum of 2 years of recent acute care case management experience. 5-7 years of clinical experience. Bachelor’s degree required. Registered Nurse (RN) licensure. Proficiency in case management and care management. Experience with Epic systems. Additional Skills & Qualifications Master’s degree in Nursing, Health Administration, or Business Administration preferred. Health information management and medical terminology knowledge.
