Manager of Case Management Job Description The Manager for Case Management services is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management within their assigned area. This position requires alignment with the goals of the Case Management Department. The Manager will oversee the case management program, providing expert clinical practice, subject-matter expertise, resource support, and leadership to the case management team. Responsibilities Provide day-to-day oversight and management of the case management program. Coordinate, supervise, and provide administrative oversight to the case management team. Function as an expert clinical practitioner and subject-matter expert for the case management team. Supervise and monitor professional and support staff. Ensure effective care coordination and case management practices align with hospital policies and regulations. Monitor quality interactions, documentation, and assessments of the case management staff. Coordinate activities that promote quality outcomes and patient throughput.
