Job Description

Position Summary We are currently seeking a Director of Case Management to oversee and lead the hospital’s case management operations. This leadership role is responsible for ensuring quality patient care coordination, effective discharge planning, and compliance with policies and regulatory standards. The Director will work cross-functionally with medical staff and executive leadership to achieve departmental and organizational goals. Essential Duties and Responsibilities Oversee all case management operations, including staffing, productivity, onboarding, and competency development Lead care coordination and discharge planning processes for quality, efficiency, and compliance Collaborate closely with CFO, CMO, CNO, and other senior leaders on department strategy and performance Analyze operational data to drive continuous performance improvement Ensure adherence to organizational policies and case management best practices Build strong relationships with internal and external stakeholders, including Legal, Ethics & Compliance, and ancillary teams Promote and uphold the organization’s mission, values, and code of conduct Knowledge, Skills and Abilities Proven leadership and communication skills Education & Experience Bachelor’s degree required Master’s degree in Nursing, Health Administration, or Business (preferred) Active RN license (LCSW not accepted) 2+ years of recent acute care case management experience 5-7 years of overall clinical experience
