Requirements:

Area of Responsibilities Communication Team Work Conduct. Documentation and Management of the Medical Record. Completes treatment plans, progress notes, and other clinical documentation in specified time limits, and oversees paraprofessional documentation to assure it is in-line with best practices and meets standards. Completes initial required paperwork for pre-certification of consumer and all subsequent paperwork as needed. Is responsible for integrating assessment information into a comprehensive treatment plan for the consumer and family. Is required to adequately document billable hours each week (set by Program Manager), and empowers, coordinates, and oversees the team’s billable hour standard, in an effort to meet weekly minimums and standards. Participates in discharge planning and referral contacts and verifies follow through and documentation of discharge plans. Documents calls and related information on designated forms. PROVISION OF CARE

Establishes and maintains that the team is operating to have positive relationships with patients, physicians, allied health professionals, and all referral sources. Works to ensure that treatment within the team is delivered in team format in collaboration and under the direction of the team lead and assures that the patient’s best interest is reflected. Seeks, identifies, and empowers the team to look for opportunities to improve care, patient flow, or utilization of resources. Participates in the orientation and supervision of new employees and students and takes on the overall responsibility to assure that all members of the team are fully oriented.
