The Director of Assessment & Referral (A&R) is responsible for overseeing the coordination of patient assessments and initiating the admission process. This role involves direct supervision of A&R Counselors and close collaboration with
Admissions Clerks Coordinating Patient Assessments:
Directing A&R Counselors in the comprehensive assessment of incoming patients.
Initiating Admissions:
Working closely with Admissions Clerks to facilitate and streamline the patient admission process.
Discharge Planning Support:
Providing technical assistance for the discharge planning process, under the guidance of the Director of A&R.
Interdepartmental Collaboration:
Collaborating extensively with Clinical teams, Utilization Review (UR), Hospital Leadership, and community resources to ensure seamless patient flow and effective care coordination. This role is pivotal in managing the initial patient journey and ensuring effective communication across various hospital departments and external partners.
PRIMARY RESPONSIBILITIES
Inquiry calls are documented and processed in accordance with Intake policies. Patients are assessed and stabilized prior to leaving hospital or admission to hospital arranged. On call doctor is notified and assisted with patient interview. Administrative approval is obtained for all admissions. Ensures admission paperwork is completed in accordance with hospital policy. Intake log is completed with all information documented. Follow up calls are made to those failing to show up for appointments. Pre-certification is obtained in accordance with insurance requirements. Outcome studies forms are completed. Guest relations are demonstrated by example and employees in need of guest relations training are counseled. Administration is notified of problems or significant events. Safety problems are identified and corrective actions taken. Medication is obtained after hours as ordered by the physician with proper documentation of drugs removed and accountability maintained. Integrity of the pharmacy is maintained with all locks secured and alarm activated. Requests for assessments are timely coordinated. In house assessments are completed with treatment or referral disposition recommendations made and documented.

Education :
Master’s Degree in nursing or a human relations field. preferred
Experience :
Two or more years experience in intake assessment activities within an acute psychiatric setting with supervisory experience. A minimum of two (2) years experience in a psychiatric hospital setting including customer contact. preferred.
Licensure :
Licensed to practice in the State of Georgia . preferred
Additional Requirements :
A working knowledge of the DSM-5 and the Mental Status Exam. A working knowledge of the Addiction and Recovery field. Compensation:
