Job Description
Responsible for region wide strategy, development, implementation, and management of the Inpatient Care Without Delay program. Care Without Delay is a patient flow oversight model that aligns multiple department processes to improve the timeliness and appropriateness of patient care through enhanced provider communication and the identification and removal of system barriers to care. An effective CWD program achieves the triple aim of high-quality care, enhanced affordability, and better patient care experience. Scope of work includes core and noncore adult inpatient care and Continuum care. Major Responsibilities/Essential Functions • (In order of importance, list primary job duties this position is responsible for achieving. Each statement should relate to an end result or objective). 1. Develops structure for oversight of entire core hospital production line looking for alignment and system barriers. Ensures that system issues are managed efficiently. 2. Develops and maintains a 24/7 escalation process that is easily activated if care/treatment/service is not delivered in the time frames established, and preferable the day it is requested. 3. Partners with Health Plan Senior Director of Hospital Utilization Management to implement principles of CWD in the core Hospital. Additionally, takes joint responsibility for performance of Inpatient Case Managers, Including input on performance reviews, goal setting, education, and onboarding. 4. Partners with the Chief of Hospitalists, the Physician Director of ECM Hub, Utilization Management, and Virtual Hospital, and the Chiefs of specialty departments to create a culture where core principles of clinical quality are known and practiced including: Timeliness of care, Real time Peer review, Real time escalation, Excellent communication and Strong collaboration with warm hand offs, Focus on Long view of care; with removal of conflicting priorities and perverse incentives. 5. Runs inpatient CWD Rounds with focus on highest quality of care and identifying and eliminating inefficiencies from admission through discharge. Rounds should be a system that feeds direct knowledge of what patients are experiencing in the core hospitals to the CWD leadership on a very frequent basis. When issues are identified at rounds, there should be real time communication and intervention with physicians and staff to drive quality and efficiency improvements. 6. Develops relationships with hospital leadership to strengthen partnership behavior and realize joint efficiencies. Understands bed capacity issues at core and noncore hospitals and bed control processes. 7. Runs monthly Complex Case Conference and Care Without Delay Committee Meeting in conjunction with Outpatient Director CWD for various groups to reinforce the principles of CWD, discuss system issues, and provide feedback on performance. 8. Develops orientation and on boarding, in conjunction with Physician Director of Inpatient CWD, the introduce CWD at MGO for new hires. 9. Partners with Director of ECM HUB, Director of Continuum and Director of Inpatient CWD to deliver our organizational CWD strategy. Develops and monitors metrics with the Continuum directors to measure performance in Continuum operations. 10. Facilitates development of Medical Service Agreements between specialty departments, educates providers on these agreements, ensure that these agreements are readily available and routinely reviewed. 11. Identifies knowledge gaps in hospitalists and inpatient case managers and assists with plans to remediate. 12. Maintains close awareness and monitoring of PDR performance and identifies trends. Participates in drill down to identify drivers of unsatisfactory PDR and helps establish initiatives to improve PDR. 13. Works to assure good relationships between physicians and case managers. 14. Maintains working knowledge of NICU utilization program. 15. Participates in identification and management of catastrophic cases. 16. Maintains knowledge of best practices for effective delivery of care in the inpatient setting and stays abreast of community practice and literature developments that may impact inpatient utilization.
