Job Description Summary:
As a Forms Coordinator an employee must be patient-service driven with excellent attention to detail, to ensure all patient submitted forms are prepared for their personal use. A successful employee must also be able to liaise with patients and coworkers, including physicians, to quickly and efficiently provide critical documentation for patients. The employee must be able to problem-solve and multitask as we are a fast-paced practice. This job is a full-time, benefited position at Summit Spine & Joint Centers that reports to the Admin Call Center Manager. This position will be based in the SSJC Administrative building in Lawrenceville, GA.
Responsibilities:
Obtain, review, and process FMLA medical certification forms for completeness and accuracy, ensuring alignment with clinical documentation and medical standards, including but not limited to: FMLA, Short Term Disability, Long Term Disability, Return to Work Letters, and Discharge Letters. Interpret medical records and diagnoses to verify appropriate justification for leave based on FMLA guidelines. Ensure consistency between clinical notes and what’s documented in the FMLA forms. Coordinate with physicians, nurse practitioners, other clinical staff, and third-party services to ensure timely completion of medical certification forms. Communicate with patients regarding any additional items or services required before a form may be finalized. Ensure that FMLA documentation complies with federal regulations, clinical policies, and legal standards. Flag and escalate cases that may not meet FMLA medical necessity requirements or require further physician input. Work accurately, work well independently and with others, and able to maintain focus under pressure. Maintain regular written and verbal communication with internal providers and third-party agencies Coordinate management to accurately document completion status of forms. Other administrative duties as assigned.
Skills And Abilities:
Must be personable and detail oriented as a representative of the practice. Excellent verbal and written skills for proper documentation of records. Proficiency with Microsoft Office software.
Education And Experience:
Working knowledge of
ICD-10, CPT
coding, medical terminology and practices. Proficiency with EMR (EClinical Works), 1+ years’ experience preferred. 1+ years of experience in a medical setting preferred.
