The function of this role will require the Associate Clinical Director to work in an office setting at the Capstone Hospice office located in Peachtree Corners. Candidates must meet the following requirements to be considered:

Education/Degree:

Graduate from accredited school of nursing

Training/ Licensure:

Currently licensed as a registered nurse in good standing in the state of Georgia.

Certifications:

CHPN certification is preferred.

Experience:

5+ years hospice experience is required with 2+ years of leadership/management experience to include direct and in-direct reports of a team, consisting of 4 or more employees. This position is supervised by the Chief Clinical Officer. This position supervises the Patient Care Manager and On Call Manager.

In-direct relationships with:

Director of clinical services and other health care team members. The essential responsibilities of this role include the following but are not limited to: Planning, directing and participating in the delivery of clinical services to hospice patients by leading the Patient Care Manager and On Call Manager. Oversee assessment, evaluation and documentation by the clinical teams through reporting and attendance in IDG for each team. Monitor reporting from the Patient Care Coordinator and ensure data is collected and reviewed with the Chief Clinical Officer weekly. Lead clinical initiatives in collaboration with the Chief Clinical Officer, Compliance and Education. Drive strong communication between teams to ensure consistency in the process. Support follow up on reimbursement concerns. Provide mentorship and guidance to clinical management staff, ensuring high quality patient care. Support growth in facilities through mentorship of PCMs, fostering positive external relationship practices. Support On Call manager in monitoring on call utilization, response time and documentation. Monitor staffing patterns and work with Chief Clinical Officer to maintain standard staffing model. Participated in QAPI Maintains skills and knowledge of hospice standards. Performs specific assignments as directed by the Chief Clinical Officer. Provide ongoing feedback, verbal and written, on team performance Complete timely annual evaluations of assigned team members.

Candidate should possess the following knowledge, skills and abilities to be successful in this role: Ability to work independently, make accurate and at times, quick decisions. Ability to handle multiple tasks and manage priorities effectively. Strong communication skills. Organizational and critical thinking, problem-solving skills with the ability to lead an interdisciplinary team. Understanding basic reporting and use of Teams, Excell and Outlook. Committed to safety precautions and high ethical standards. Understanding and skilled in hospice assessments. Proficient in English (understanding and articulation). Cool-tempered and patient. Emotional/mental stability.

JOB FACTORS
Physical Requirements:

Requires considerable physical presence in office setting most days. Ability to travel to community settings, possessing sight/ hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state-required tests or examinations.

Mental Requirements:

Must be able to work independently, make judgements based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:

Essential functions of the position require working in an office setting with exposure to lighting and heating/air systems.

Transportation:

Must have a current valid driver’s license, auto liability insurance and reliable transportation. This is a full-time, salaried-exempt position.
