Job Overview:

The Patient Intake Coordinator manages incoming referral calls, initiates the intake process for new patients, and supports seamless communication between patients, providers, and care teams. You will coordinate appointment scheduling, verify essential intake information, and provide excellent customer service to patients and caregivers—all while contributing to an efficient, responsive healthcare experience.

Key Responsibilities:
Patient Intake & Referral Coordination:

Answer and triage incoming calls from patients, referral sources, and providers with professionalism and empathy. Gather and document critical intake information, including patient demographics, referral details, and care needs. Communicate with patients to explain the intake process, confirm appointment details, and answer initial questions. Track referral status and ensure timely follow-up with referring providers and internal care teams.

Scheduling & Communication:

Coordinate and confirm wound care appointments, ensuring timely scheduling aligned with patient needs and provider availability. Liaison between patients, family members, and clinical staff to ensure clarity and comfort throughout the intake journey. Maintain accurate records within the electronic health record (EHR) system in compliance with HIPAA and company protocols.

Team Collaboration & Support:

Work closely with Patient Services Coordinators, clinical staff, and leadership to ensure a seamless transition from intake to care delivery. Support the administrative flow of patient documentation and onboarding materials. Participate in continuous improvement efforts to enhance the intake process and patient experience.

Qualifications:

A bachelor’s degree is preferred; a degree in healthcare administration, public health, or a related field is a strong plus. 3-5 years of experience in a healthcare, front office, patient intake, or care coordination role, preferably in a fast-paced medical setting. Outstanding interpersonal and verbal communication skills, with a compassionate, patient-first approach. Strong organizational skills and the ability to multitask while maintaining attention to detail. Proficiency in Google Workspace (Docs, Sheets, Gmail) and experience with electronic health record (EHR) systems. Ability to navigate sensitive conversations with empathy, confidentiality, and professionalism.

Preferred Characteristics:

Experience working in transitional care or senior care settings. Ability to remain calm and solution-oriented under pressure. A proactive learning and personal growth approach, especially in emotional intelligence and patient engagement. Strong organizational skills with a focus on timely follow-through.
