Duties:
Manages and directs Case Management Supervisor, Social Workers and Discharge Planner Develop and maintain a good working rapport with personnel within the facility and outside community health, welfare, and social agencies to meet the needs of the patients and their families. Will oversee the preparation of care management reports presented by the Case Management Supervisor to the Hospital Activities Committee with recommendations for interventions when needed Will facilitate relationship between medical staff to ensure the proper status of patients admitted to Lane Regional Medical Center Identify trends with delay in care, increase LOS, over or under utilization of resources, discharge planning and work with staff to address trends identified Responsible for all policy and procedures for social services including staffing plans and scheduling Licenses & Certifications Required Registered Nurse Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
