Responsibilities:
Manages and oversees the DaVita IKC specialty operations teamfor IKC commercial lane Ensures compliance to all applicable regulations and DaVitapolicies Monitors risk management and quality assurance compliance Measures patient care outcomes relevant to the specialtyoperations lane Meets financial goals established for membership enrollmentspecific to the specialty operations lane Builds and maintains effective relationships with teams. Actively participates with cross-functional teams to drive onspecialty initiatives Coordinates activities with DaVita IKC management team,providers and cross-functional departments, and others asneeded Maintains and manages relationships with key constituencies,including providers and health plans Creates a special place for teammates. Supports leadership in the development and expansion ofinitiatives to innovate around complex patient management. Serves as clinical SME on projects and initiatives wherespecialty operations is key to innovation success. Identifies opportunities for development and expansion ofenhanced or new services Other duties as assigned
Education:
4 to 6 years of relevant experience Bachelor of Science Nursing or Master’s Degree in Social Workpreferred Active Licensed Clinical Social worker or Licensed ProfessionalNurse with no disciplinary action required Clinical practice experience a minimum of 5 years in:
Palliative care/hospice care or Renal care experiencepreferred Intermediate computer skills and proficiency in MS Word, Excel,PowerPoint, & Outlook required Deals with confidential information and/or issues usingdiscretion and judgment preferred Certification in: Case Management (CCM), Nephrology Social Work(NSW-C) , Nephrology Nursing (CNN) preferred Up to 25% travelWhat We’ll ProvideMore than just pay, ourDaVita Rewards package connects teammates to what matters most.
