Job Description
Clinical Director Neulife Rehab – Parent Account 41238 Adams Rd, Hammond, LA 70403 NeuLife Rehabilitation (NLR) provides post-acute rehabilitation and residential services for individuals with Traumatic Brain Injury (TBI), Acquired Brain Injury (ABI), Spinal Cord Injuries, and Neurological Disorders. Summary The Registered Nurse/Clinical Director is responsible for advancing community integration by managing, directing and providing medical treatment to patients with neurological injuries and or impairments, following the established plane of care to ensure patient progress for effective outcomes, and improving the patient’s quality of life. Provides patient with advanced care and medical treatment in accordance with the care plan and qualifications. Provides patient with advanced care and medical treatment in accordance with the care plan and qualifications. Completes health exams assessments upon admittance, return from hospital and/or as needed. Assists in development of care plan for patients and oversees administration of the care plan including reviewing plans of care monthly, quarterly and/or as needed. Follows all medication procedures established in the care plan including overseeing medication management program, proper dispensing and dosage, and communicating with pharmacy staff as needed. Communicates with internal staff members as well as outside members of the treatment team to include primary care physicians and specialists. Completes medical records, reporting and charting in a timely manner. Works collaboratively with other members of the treatment team. This includes being available in person or via phone for scheduled team conferences. Follows all HIPAA and patient confidentiality requirements at all times. Participates in emergency and on-call work as scheduled. This includes being available by telecommunications and/or able to be on site as needed 24 hours/day. Provides training to patients related to medication, treatment and self-care. Other duties as assigned.
Certificates, Licenses, Registrations, Qualifications:
EDUCATION/CERTIFICATION
Graduation from an accredited RN program. Current state RN license in good standing. CPR/First Aid Certification.
REQUIRED KNOWLEDGE
Working knowledge of rehab facility operations, medical terminology, anatomy, physiology, computers, and office equipment.
EXPERIENCE REQUIRED
One to three years of related experience.
