Position Summary
Provide resource and referral guidance, case management, and therapeutic matching to participants within the juvenile court clinics (JCC) of Suffolk and Norfolk Counties.
Essential Functions
Research and identify programs and providers for court involved youth, parents and families experiencing a wide variety of needs.
Provide triage and psycho-education to participants who are seeking coordinated therapeutic or natural support matches for mental health and wellness services
Serve as coordinator on JCC referral cases as scheduled and assigned by program director/clinical supervisor
Provide follow up services and case management to assigned cases
Work with Court personnel such as probation officers, attorneys and judges to coordinate program referrals
Liaison with state agencies (DMH, DCF, DDS, DPH, DYS) by attending meetings and phone conferences to determine best practices for resource and referral for participants in the juvenile court clinics
Address obstacles to accessing services for participants in the court clinics in coordination with referral sources and other staff of the clinics.
Work on special projects/programs/committees reflecting the emerging needs of the court such as JDAI and Dually Involved/Crossover Youth at the request of program director/ clinical supervisor
Maintain resource database and detailed records regarding identified community programs
Attend trainings and staff meetings as necessary and appropriate
Train, manage, and supervise student trainees as assigned by the program director/clinical supervisor
Other duties as assigned
Education and Experience
A minimum of an associate’s degree or Bachelor’s degree in clinical psychology, social work or related field
Skill in both verbal and written communication
Strong interpersonal and communication skills
Experience with triage in a clinical setting and knowledge of social service systems in Suffolk and Norfolk county
Preferred lived-experience with Juvenile Court, DCF, DYS and other child-serving agencies
