Assisted Living Unit, Resident Service Coordinator Alliance Health & Human Services 144 Turnpike Road, Southborough, MA 01772 Assisted Living Unit- Resident Service Coordinator This job description, defining basic responsibilities, basic functions, general responsibilities, and duties listed here are not to be considered all-inclusive. This job position performs these and other duties as may be assigned by his/her supervisor.

Professional Requirements:

Graduated from Accredited School of Nursing. Is licensed and in good standing in Massachusetts as a Registered Nurse (RN) or Licensed Practical Nurse (LPN). Has at least (2) years experience and clinical management experience. Is experienced with Executive Office of Elder Affairs Regulations as they pertain to Assisted Living. Is experienced working with and has formal training in the care and treatment of persons diagnosed with Alzheimer’s disease or related dementia’s. Meets facility expectations for employment and personal references. Meets facility criteria for license and criminal background checks (CORI).

Physical Qualifications:

Has a valid employment physical and tuberculosis test. This position requires that the employee be able to perform the stated job functions. The applicant must pass all pre-employment exams and be in good physical and mental health. This job requires frequent bending, walking, and lifting. Occasional assistance is required to help with resident care. Minimum lifting requirement of (50) pounds for this position. You must also have complete Range of Motion. You must notify your supervisor immediately if you have any health restrictions.

Responsibilities:

To operate the Assisted Living Unit in accordance with established policies and procedures of the governing body. Is responsible to perform Pre-Admission, Initial, and Ongoing Assessments of residents. Is responsible for developing and timely review of Resident Service Plans. Is responsible for the Medication Management of the assisted living residents. Provides the necessary initial and ongoing training to staff as required. Maintains good communication with staff, residents, family members and the governing bodies. To address concerns, disputes, and complaints quickly and fairly using facility policies, state regulations, and other pertinent facts. To provide guidance and support to residents, and families regarding continued residency and coordination of outside serves. To ensure that resident needs are met and that the service plan reflects those needs. To ensure that all documentation is accurate, comprehensive, and legible. To maintain a safe working environment. To assist with resident care as needed. To perform initial and annual performance reviews of the personal care attendants (PCA). To provide appropriate staffing of the unit at all times. To complete employee work schedules in advance and maintain appropriate staffing levels at all times. To be on-call as required by your supervisor.
