Reliant Medical Group (Primary Care for Adult Medicine) has an immediate opening for a friendly, patient focused and detailed oriented Associate Patient Care Coordinator to join our team. The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.

Maple Ave Shrewsbury, MA Hours:
Monday through Friday, between the hours of 7:30am – 5pm EST. Schedule will be determined upon hire.
Primary Responsibilities:
Primarily call center environment in a patient clinic team-based setting
Handling phone incoming and outgoing phone calls
Message management in EMR (electronic management record) system
Providing direct administrative support to the practice and within a care team
Scheduling appointments
Organizing and completing patient forms and letters
Updating patient information
Insurance verification
Obtaining prior authorizations for exams/testing
Collaboration with medical providers, nurses, medical assistants and ancillary staff
Problem solving patient issues/requests

Required Qualifications:
High School Diploma/GED (or higher)
1 + years of customer service, call center and/or healthcare experience
Preferred Qualifications:
1 + years of experience working in a healthcare setting (i.e. Urgent Care, Specialty)
Experience with EPIC/electronic medical record systems
Basic level of knowledge of Microsoft applications (i.e. Excel, Word, Teams)
Working knowledge of general office procedures
Bilingual English/Spanish Soft Skills:
Strong communication and customer service skills both in person and via phone
Self-starter with effective multitasking skills in a fast-paced environment •
