The AssociatePatient Care Coordinator (Primary Care) coordinates the provisionof multiple services to patients performing a full range ofadministrative support services for clinic/site includingcollection of co-payments, registration, and scheduling of patientsfor medical procedures, tests, associated ancillary services andother related duties to ensure the highest level ofpatient/customer satisfaction.

ESTPrimary Responsibilities:
  • Verifies, re-verifies, and authorizespatient insurance coverage and eligibility utilizing computer-basedpatient registration/scheduling system
  • Verifies and updatesdemographic, insurance and other patient information
  • Confirms,collects, and posts patient co-payment and other outstandingbalances
  • Generates and tracks patient encounter forms; reviewspatient encounter forms for completeness and resolves anydiscrepancies. Batches encounter forms and submit to appropriatebuilding area
  • Receives and addresses patient care relatedtelephone calls. Provides routine information on procedures andstandard policies, refers matters to appropriate person withdepartment/site
  • Identifies problem related priorities andresponds to emergency needs by contacting/interpretingphysician/provider within guidelines
  • Resolve patients’ questionsregarding clinic schedules and billing concerns; handles requestsfor prescription refills, reschedules appointments, etc.
  • Documents appropriate information in computer system
  • Preparepatient charts
  • Ensures completeness by locating test results,reminding patients of appointments, including appropriate forms,etc., for patient appointments
  • Updates and maintains TelephoneMessage System (TMS) in computer system. Follows through onrequests. Distributes messages to appropriate provider or othersfor appropriate action
  • Enter new referrals or ensure thatexisting referral numbers are linked in the system to ensuremanaged care requirements
  • Provides patient education regardingmanaged care plans and referral process. Answers patient’s referralquestions, concerns, etc. Provide patients with available options
  • Works collaboratively with primary care practices, patients, andspecialty practices to process outgoing referrals prior toscheduled visits. Follows up with practices that do not issue areferral or obtain referral on a timely basis
  • Interfaces withstaff, providers, and patients regarding referrals denied.

Documents information and assists with alternate plan of care, ifneeded

  • Completes and maintains patient schedules. Schedules andcoordinates patient visits and medical procedures for bothinpatients and outpatients. Communicate with patients regarding allinformation related to scheduled appointments
  • Notifies providers,patients and others of changes such as new scheduling,re-scheduling, no-show, emergency appointments and add-ons
  • Maintains recall lists and communicate with patients asappropriate. May inform nursing staff or others of laboratory anddiagnostic study results; collects and mails test results
  • Mayprocess standard and non-standard business office administrativepaperwork, such as purchase requisitions, employee timesheets,expense vouchers, etc. Follows up with personnel outside themedical office to expedite timely action and alleviate, or reportdelays as appropriate
  • Provides guidance to departmental personnelin the medical office on administrative policies and procedures
  • Comply with health and safety requirements and with regulatoryagencies such as DPH, etc.
  • Comply with established departmentalpolicies, procedures, and objectives
  • Enhance professional growthand development through educational programs, seminars, etc.
  • Attends a variety of meetings, conferences and seminars as requiredor directed
  • Regular, reliable, and predictable attendance isrequired

Required Qualifications:
  • High SchoolDiploma/GED (or higher)
  • 1 years of customer service experience
  • Intermediate level of proficiency with general computer use
Preferred Qualifications:
  • Specialized medical secretarial/medicaloffice training
  • Healthcare experience
Soft Skills:
  • Ability tomultitask and resolve patient concerns in a timely manner
  • Abilityto think critically and exercise sound judgement

The salary range for this role is $16.00 to $28.85per hour based on full-time employment.
