Job Summary:
Overall, the role of the inpatient care manager is to optimize the quality and continuity of care for patients during their hospital stay, with a focus on promoting positive health oues and patient satisfaction.
Job Description:
The Role:
Inpatient Care Manager (RN or LSW)
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Position Highlights:
This position is either 32 or 40 hours per week with weekday hours. T is an occasional weekend rotation.
What You’ll Do:
Interviews and assesses patients and/or patient’s family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Consults with multidisciplinary teams regularly to evaluate patient’s status and appropriateness of medical care, including coordination of care in all care centers.
Participates in discharge planning including identification and coordination of appropriate resources. Provides discharge education and resource referrals to patients when necessary.
Performs chart review to identify actual or potential issues with service delivery, patient oues and satisfaction,pliance, cost, and reimbursement.
Your Qualifications:
Maine state licensure as a Licensed Social Worker (LSW) or Registered Nurse (RN) is required
Excellent teamwork,munication, and collaboration skills
Proven and demonstrated critical thinking skills
