ABOUT THE ROLE

Under the direction and supervision of the Community Health Director, the Community Health Coordinator is responsible for providing support and guidance to program Lifestyle Coaches, participants and potential participants within the YMCA Chronic Illness programs. The Community Health Coordinator will also support relationships with key partners, participating with screening events and driving referrals to the Community Health Worker Team and YMCA diabetes-focused Evidence Based Health Programs. This position is contingent upon continued grant support, 15-25 hours per week with some hours being nights and weekends.

ESSENTIAL FUNCTIONS

Models inclusive behaviors and embraces all dimensions of diversity Participates in the budgeting process within their service line/department and serves as a good steward of association resources Apply a results-driven approach to ensure the service line/department aligns with association-wide goals, regularly tracking progress against established KPIs. Support the implementation of Diabetes Prevention Programs (DPP), including participant recruitment, referrals, community outreach (e.g., health fairs, screenings, and marketing), Lifestyle Coach assistance, Community Health Worker support, incentive coordination, and data reporting. Assist in fostering partnerships with key collaboration organizations and subject matter experts to promote health equity and improve outcomes for underserved populations. Establish and nurture relationships with community members, healthcare providers, and local community-based organizations to build a robust referral network. Guide prospective DPP participants through enrollment processes, class placement, paperwork, and lead departmental event participation. Deliver outstanding, culturally competent customer service while engaging with participants, YMCA staff, members, and diverse community stakeholders. Develop strong, authentic relationships with community partners and participants by actively listening, showing empathy, and responding to their needs. Maintain a professional and approachable demeanor with a willingness to learn and grow. Complete training to become a Lifestyle Coach for the National Diabetes Prevention Program/Peer Educator for the Diabetes Empowerment & Education Program, with the goal of coaching in the future. Collaborate with the Food Access team to align services, strengthen program integration, and assist with day-to-day operations as needed. Perform other duties as assigned in support of program and organizational goals.

ABOUT YOU

Qualifications At least two years professional experience in community-based work in program coordination, leadership, and/or community engagement. Bilingual in English and Spanish is highly preferred. Proficiency with Microsoft Office Suite (Word, Excel, Outlook, Teams) and comfort with learning other digital tools. Access to reliable transportation and willingness to travel across three counties approximately 3-5 times per month. Willingness and availability for occasional nights and weekends. Community Health Worker (CHW) or Health Coaching certification preferred but not required. Knowledge, Skills, and Abilities Excellent personal computer skills and experience with standard business software. Requires strong organizational skills, prioritization of workloads in order to meet deadlines, as well as detail orientation and conscientiousness. Must have excellent interpersonal, public relations, and communications skills with a strong understanding of participants’ cultures, values, and health practices/ beliefs. Comfortability in both office and community-based work environments. This role is a combination of in-person and home/office-based work.
