The Home Health Patient Care Manager isresponsible for the supervision and coordination of clinicalservices and provides and directs provisions of nursing care topatients in their homes as prescribed by the physician and incompliance with applicable laws, regulations, and agency policies

• Provides clinical services within the scope of practice, asdefined by the state laws governing the practice of nursing, inaccordance with the plan of care, and in coordination with othermembers of the health care team.
• Receives referrals, ensuresappropriate clinician assignments, evaluate patient orders, andplot start of care visits.
• Coordinates determination of patienthome health benefits, medical necessity, and ongoing insuranceapprovals.
• Ensures patient needs are continually assessed andcare rendered is individualized to patient needs, appropriate andreasonable, meets home health eligibility criteria, and is inaccordance with physician orders.
• Reviews assessments and plansof care daily, per assigned workflow, and consults clinicians withrecommendations, as appropriate. Education & Experience
• CurrentRN licensure in state of practice
• Current CPR certificationrequired
