GENERAL DESCRIPTION OF DUTIES

The primary purpose of this position is to manage and oversee the Supporting Women’s Health Services Agreement Addendum (AA175) to help lower the unintended pregnancy rate, overall infant mortality rate, and/or the maternal mortality rate in Duplin County. This includes planning and organizing group sessions, participating in community outreach events, and helping to recruit participants into the program through direct interactions, social media, and other forms of outreach. Work is performed under the general supervision of the PHN III Women’s Health Program Manager. The six available Evidence-Based Strategies (EBSs) are: Extended Clinic Hours, Satellite Clinic Locations, Group Prenatal Care, Home Visit for Postnatal Assessment, Community Health Worker Integration, and Behavioral Health/Maternal Mental Health Providers. EBSs:

Group Prenatal Care and Community Health Worker Integration. Group Prenatal Care is a model of group care where pregnant individuals, who are due at about the same time, meet with their provider and other pregnant individuals for group sessions over the course of their pregnancy. Group sessions are 90 minutes to 2 hours in length and consist of health assessments, facilitated group discussion and interactive activities, and education on timely pregnancy, birth, and postpartum topics. Data supports that women have experienced positive birth outcomes and lower racial disparities for low birth weight and preterm births by participating in the group care model. Community Health Workers are trusted members of the community served and/or have a close understanding of the community served, which enables the CHW to serve as a liaison/link/ intermediary between health/social services and the community. This also enables the CHW to facilitate access to services and improve the quality and cultural competence of service delivery. CHWs are qualified to work with individuals of reproductive age, their children, and families in efforts to improve community outcomes for maternal and infant health.

SPECIFIC DUTIES AND RESPONSIBILITIES ESSENTIAL FUNCTIONS

  • Assist with providing health assessments, facilitating interactive group discussions, and providing education and materials on health topics.
  • Supervises group prenatal care scheduling and follow-up on missed appointments.
  • Coordinates with the Maternal Health Team to ensure lab results have been ordered, reviewed, and followed up on the group prenatal care participants.
  • Coordinates with the Maternal Health Team to schedule referrals for the group prenatal care participants as needed.
  • Maintains all group prenatal care participant records in the electronic system and any paper documents as indicated.
  • Ensures health educational and informational materials are developed at or below the sixth-grade reading level and in a culturally and linguistically appropriate manner.
  • Coordinates health education materials for the group prenatal care program.
  • Establishes and maintains confidential records according to agency policy and HIPAA.
  • Monitors the budget and supply needs for the AA 175 and coordinates the ordering of approved supplies.
  • Collaborates with the Maternal Health Team when scheduling group prenatal care participations for ultrasounds.
  • Promote breastfeeding, including support and education.
  • Integrate the Community Health Worker into the group prenatal care program.
  • Ensure promotion of the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) services.
  • Ensure appropriate referrals are made for high-risk pregnant women to Care Management for High-Risk Pregnancy (CMHRP).
  • Ensure promotion of other health and human services/resources on behalf of the participant such as 12-month postpartum Medicaid coverage.
  • Responsible for the performance measures and reporting requirements, including conducting group prenatal care annual program reports, monthly itemization reports, participant satisfaction surveys, appropriate budget utilization, program goals, objectives, and outcomes.
  • Conducts at least one group serving pregnant individuals meeting for 10 sessions, which follows the recommended schedule of prenatal care visits.
  • Assures reporting of required data to the Women, Infant and Community Wellness Section consultant (WICWS) as requested.
  • Collects data on program participants and reports on a quarterly basis. This includes demographic information, prenatal care visits, breastfeeding initiation and duration, delivery outcomes, and birth outcomes, as well as programmatic data for those receiving CHW services.
  • Develop and administer a participant satisfaction survey to evaluate services and submit an annual satisfaction survey summary report. The patient satisfaction survey should measure the patient’s expectations for care and whether they were met.
  • Distribute a patient experience survey developed by the WICWS to patients assessing the group prenatal care model.
  • Collaborates with other agencies in the community regarding issues related to maternal health care.
  • Represents DCHD on community boards and/or committees and promotes available services.
  • Promotes quality care, use of motivational interviewing, accurate, concise, and complete documentation of services rendered.
  • Stays informed of current trends in public health by attending continuous education opportunities that promote professional and personal growth.
  • Coordinating training for new and/or reassigned program staff in the selected models.
MARGINAL FUNCTIONS

While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members. 2% Other Duties

  • Participates in shelter duty and other activities relating to the Department’s emergency response and other disaster relief efforts.
  • Serves as a team member during outreach projects such as flu vaccine season and disease outbreaks.
  • Performs any other duties and responsibilities as directed.
MINIMUM TRAINING AND EXPERIENCE
  • Bachelor’s degree from an appropriately accredited institution; or Equivalent combination of training and experience.
SPECIAL REQUIREMENTS
  • Proficiency with Microsoft Office tools.
  • Strong public speaking and presentation skills.
  • Must have a valid NC driver’s license.
  • Reliable means of transportation for work assignments.
