FULL JOB DESCRIPTION PRINCIPAL DUTIES & RESPONSIBILITIES
- Provides daily supervision of Certified Nurse Aides and ensures compliance with Adult Day Health (ADH) and local regulations.
- Orients and trains staff, volunteers, students, and contractors.
- Oversees participant care, directs nursing assessments, and ensures timely follow-up with PCPs and IDT.
- Acts as ADH Health Care Coordinator, ensuring policies, procedures, and compliance are maintained per state and federal regulations.
- Serves as liaison between staff and administration, supports site audits, manages vendors, equipment, supply ordering, and departmental budget.
- Leads care planning, supports CNA participation in IDT, and ensures documentation and service delivery meet standards.
- Collaborates on quality initiatives, participates in meetings, and supports policy development and caregiver education.
- Participates in on-call rotation and ensures high standards in daily operations including engagement, meals, and personal care.
REQUIREMENTS
- Active NC Registered Nurse license required; BSN preferred.
- Minimum 5 years nursing experience, including 2 years in management and 1 year working with aging/frail populations.
- Preferred experience in home care, long-term care, or PACE programs.
- Must demonstrate leadership, problem-solving, and critical thinking.
- Strong communication, teaching, and team collaboration skills required.
- Must meet
CMS/PACE
competencies. Ability to lift 50+ lbs, assist with mobility, personal care, tube feedings, shaving, non-intact skin, and clinic procedures.
- Must have a valid NC driver’s license and reliable transportation.
- Ability to work independently and professionally within scope of practice and agency standards.
