Behavioral Health Care Manager 3.4 3.4 out of 5 stars Kinston, NC PORT Health’s mission is to improve the lives, health and well-being of individuals and families dealing with substance use and mental health disorders. We are seeking a compassionate, caring, and dedicated Behavioral Health Care Manager to join our collaborative, multidisciplinary Tailored Care Management (TCM) team in L enoir and Duplin counties . designed to provide whole-person, integrated health care. What You’ll Do PORT Health’s Tailored Care Management (TCM) team provides whole-person, integrated health care through evaluation, assessment, care planning, coordinating, tracking, and monitoring outcomes of individuals served. The Care Manager focuses on closing gaps for physical and mental health, social determinants of health, and other non-health related needs. This position travels within Lenoir and Duplin counties. How You’ll Benefit You’ll earn a competitive salary between $51,000 and $53,000 depending on skills and experience . This full-time position offers a flexible schedule Monday-Friday from 8 am-5 pm. We also require the following:
