Position Summary Deliver direct, quality patient care, utilizing a basic clinical skill level and knowledge base. Develop, implements and evaluates/revises the interdisciplinary plan of care. Facilitates and coordinates best practice standards for Transplant and Living Donor patients throughout their transplant journey Manages all aspects living donor evaluations and transplant recipient evaluations. Provides direct patient care to post transplant patients in the clinic and coordinates their post operative care. Provides patient education for all patients seen in the transplant clinics and/or their primary caregivers. Participates in Research/Quality Improvement and patient safety initiatives for the living donor and transplant population. Ensures compliance with living donation/transplantation and Transplant improvement activities as required by CMS, UNOS and JCAHO accreditation. Establishes and maintains collaborative working relationships with patients, families, providers, and team members. Responsibilities Demonstrates understanding of living donor and recipient selection criteria. Understanding of absolute and relative contra-indications to live donation and transplantation. Recognizes need of donor/recipient informed decision, and necessity of confidentiality. Ensures process of informed choice has been followed according to ECU Health policy, transplant center protocol and UNOS requirements. Applies knowledge of growth, development, and educational and cultural background during live donor candidate/recipient candidate and family interactions. Identifies donor/recipient learning needs, coping skills, decision making process, long term expectations and commitment regarding the donation/transplantation process.
Coordinates with multidisciplinary team to provide age specific, direct patient care during the transplant or donation process and post transplant period. Provides education to living donors, transplant recipients and families about the evaluation, donation and post operative period and documents according to ECU Health policy Identifies patient learning needs and provides the necessary education. Collaborates with medical providers to identify and schedule testing required to complete evaluation for transplant according to ECU Health policy.
Understands the protocols related to transplant recipient, living donor and post transplant patients Verbalizes understanding of transplant protocols and serves as a resource for internal and external healthcare providers. Collects and reviews evaluation results with members of the transplant team and participates in the various transplant committee meetings. Documents deviations to protocols as needed.
Changes practice behavior as a result of QI data. Identifies opportunities for improvement. Participates in problem resolution activities. Participates in data collection per program QI plan. Demonstrates understanding and documentation of variances.
Serves as the primary point of contact for transplant patients (pre and post) and living donors patients during the transplant and post transplant process. Ensures communication with primary physicians, other healthcare providers and payers related to evaluation outcomes. Documents communications as appropriate in the EHR.
Demonstrates competency in basic nursing practice and mastery in the use of the nursing process. Current RN licensure in the state of North Carolina or temporary North Carolina licensure or meet requirements to hold RN licensure in a compact state.
Participate in patient education related to dialysis modality and/or access Gathers and reviews medical records related to the surgical referral. Communicates with nephrology office staff regarding access plan. Schedules appointment with surgeon for pre-access evaluation. Schedules vein mapping and diagnostic tests as ordered by provider. Follows up on referrals made to VIR and provides necessary supporting documentation. Schedules surgery for access placement and follow up appointments post procedures.
– Collaborates with the OR scheduling team to schedule cases for the surgeons.
– Monitors physician block times and schedules accordingly and releases unused time. Reviews post-operative instructions with patients in both pre and post access. Maintains appropriate documentation in the EHR. Identifies patients that are candidates for transplant evaluation and completes the referral. Supports the transplant clinics as needed. Minimum Requirements BSN preferred; ADN with a minimum of two years of medical/surgical experience. Current valid NC Nursing License required. More than 2 years of previous experience in a health care setting is necessary. Work experience with ESRD or transplant patients is preferred.
