Job #959-1 Open Until Filled Located in the heart of North Carolina’s Piedmont, Rowan County is attractively and conveniently nestled between the state’s largest city and its Capitol. Rowan County is made up of more than 20 departments and nearly 1,250 employees that are committed to providing a wealth of services to County residents and businesses. The County is currently accepting applications for the position of Care Management Supervisor—CMARC (Care Management for At Risk Children). This position performs supervision and guidance for Registered Nurses and Social Workers providing intermediate professional care management services, education, community referrals and support to families with children ages 0-5 with identified risk factors per PHP (Prepaid Health Plans) determination residing in Rowan County. Ensure quality services are consistently delivered to the target population and that care managers have all the tools they need to do their job successfully. Collaborates with the NC Division of Public Health (NCDPH), Pediatric Medical Homes, and other community partners to ensure program performance and quality measures are being met; including but not limited to: Overseeing and implementing quality improvement processes for Care Management for At Risk Children; maintaining up to date knowledge of program guidelines, policies, procedures, and update staff as applicable; and establishing and maintaining relationships with Prepaid Health Plans, pediatric offices in Rowan County, community partners, and attend regular meetings Requires thorough knowledge of and skill in the application of theory, practices, principles, and techniques employed in the field of public health and related programs; considerable knowledge of and skill in obtaining a health history and performing patient assessments sufficient to determine if there is any deviation from normal; strong knowledge base in maternal and postpartum issues; considerable knowledge of current social and economic problems relating to healthcare; considerable knowledge of available community resources and organizations; ability to plan and work collaboratively with community partners; and ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions Minimum qualifications include graduation from a four year college or university with a Bachelor’s Degree in Social Work, a Master’s Degree in Social Work, a B.S. Degree in nursing, Master’s Degree in a human service field with two or more years of care management/case management experience working with the specific population of low-income, pregnant individuals and/or children ages 0 to 5 years, Bachelor’s Degree in a human service field with five or more years of care management/case management experience working with the specific population of low-income, pregnant individuals and/or children ages 0 to 5 years, or Bachelor’s Degree in a human service field with three or more years of care management/case management experience with the specific population and has certification as a case manager (CCM preferred). If RN, must be licensed to practice as a Registered Nurse by the North Carolina Board of Nursing. Non-BSN nurses must also complete the Introduction to Public Health Nursing within the first 6 months of employment. Non-applicable to SW. Degree/Coursework must be from an appropriately accredited institution. A valid driver’s license is required. Starting Salary $57,300 – $63,000 annual (commensurate with experience) and excellent benefits package available.
