Director of Case Management About the Role Our client, a well-established healthcare organization, is seeking an experienced Director of Case Management to lead the Case Management unit at Rutherford Regional Health System in Rutherfordton, NC. This role is pivotal in ensuring effective clinical resource management, discharge planning, and patient advocacy while driving strategic initiatives to optimize care coordination and streamline healthcare transitions. Key Responsibilities Oversee the Case Management unit, including clinical resource management and discharge planning.
Lead care coordination activities in alignment with organizational policies and patient needs.
Collaborate with managers, physicians, and community healthcare workers on complex care management issues.
Work closely with departments and post-acute service providers to facilitate smooth patient transitions post-discharge.
Participate in strategic meetings at the facility, service area, and corporate levels to enhance accountable areas.
Assist in developing departmental goals that align with the organization’s strategic objectives.
Evaluate patient care quality and coordinate services between patients, staff, and physicians.
Foster a professional growth environment and promote evidence-based practices.
Educate physicians and staff on Case Management standards and regulatory compliance.
Ensure an effective Utilization Review Process and optimize care coordination across the care continuum. Must-Have Qualifications Associate’s Degree in Nursing (Bachelor’s or Master’s preferred).
Current RN license in North Carolina or multi-state compact license.
BLS Certification (ACMA Certification preferred).
CCM Certification required within the first year of employment.
At least 3 years of RN experience in acute care or care coordination.
3+ years of management experience preferred.
In-depth knowledge of care management, discharge planning, and Medicare/Medicaid requirements.
Strong communication skills, particularly when engaging with medical staff. Preferred Qualifications ACMA Certification.
Experience with InterQual criteria.
Knowledge of finance and budgetary processes.
