TCM Care Manager Supervisor Job Description The primary responsibilities of the Care Manager Supervisor are being responsible for the daily oversight of the care management team by providing clinical direction, monitoring of all care management services. Responsible for direct supervision and oversight of all TCM staffing by conducting case management reviewing, care coordination, case management and other supportive functions. Assist in carrying out the plans and daily operation of the TCM team.

Duties and Responsibilities:

Work directly with the Director to ensure the highest degree of services are provided by our TCM staff. Oversee the day-to-day TCM team operations Oversee the CIN system is being fully utilized by the staff for complete documentation. Monitor staff workloads/assignments and maintain an individual caseload based on agency needs Provide leadership, supervision, coaching, and mentoring to staff, fostering a culture of excellence in patient care Ensure staff are well trained and understand their roles Monitor documentation and oversee clinical care to ensure regulatory standards and/or contractual requirements are met Actively establish and promote a positive, diverse and inclusive working environment that builds team trust Monitor Medicaid requirements Develop and implement policies and procedures to enhance operational efficiency. Interview potential staff and coordinate their orientation and training on the TCM team Demonstrate respectful, supportive leadership that increases the success and cohesiveness of the team Conduct annual staff reviews, set goals for performance that is in line with organizational goals, and effectively communicate feedback. Provide ongoing coaching and mentoring Cultivate and encourage efforts for team collaboration and partnership Meets revenue, margin, and collection goals. Work with HR and Director to maintain and retain a highly qualified and well-trained workforce Work with HR to handle any staff performance issues

Qualification/Education/Experience:

A master’s-level fully Licensed Clinical Social Worker (LCSW), fully Licensed Clinical Mental Health Counselor (LCMHC), fully Licensed Psychological Associate (LPA), fully Licensed Marriage and Family Therapist (LMFT), or licensure as an RN; AND Three years of experience providing care management, case management, or care coordination to the population being served Experience with Managed Care Knowledge of and Experience working with Medicaid funded programs. Leadership qualities that inspire trust, respect, and motivation among staff members. Great communication and interpersonal skills when dealing with staff Problem solving skills when resolving administrative issues and conflicts. Detailed knowledge of policies and regulations in the clinical field Strong analytical skills for assessing performance metrics and implementing quality improvement initiatives. Establish performance goals and deadlines that align with the organization’s vision. Guarantee care management activities cater to the needs of special health care populations. Participate in the agency’s 24-hour coverage schedule, offering services, consultation, or referral as needed during emergencies. Hold weekly meetings with TCM staff to monitor billable documentation, contact calls, assessments, and care plan implementations, by ensuring compliance and quality of services. Travel to community locations, other agencies, and emergency departments to assist with outreach as necessary to meet members’ needs. Complete all required trainings MCO’s, AHEC, as defined by service rules, accrediting bodies, participate actively in all TCM Zoom trainings and meetings and clinical team meetings. •
