You will report to the Chief Operating Officer. Coordinating and evaluating appropriate care with efficient use of resources. Ensures compliance with state and federal home health regulations. Responsible for cost-effective, efficient care to the home care patient, with best outcomes. Ability to effectively communicate, problem solve, determine adequate staffing to meet patient needs. Acts as a role model and exemplifies the agency mission and values.
Hours:
40 hours per week, typically Monday – Friday; Administrative On Call rotation (5PM-8AM, and we generally alternative weeks of Agency Holidays). Why Work For Us? Responsibilities:
Supervises a multidisciplinary team of nurses and therapists Directs and monitors the care provided and assures quality patient care and best practices are followed Manages the daily operations being effective and efficient; makes recommendations to director to change any processes Meets with teams and department staff to review outcomes, quality measures and improvement opportunities Attends quality meetings and works collaboratively with the quality department in reviewing deficiencies, educating staff and engaging staff in process improvement activities Monitors staff productivity and meets with staff falling below standards Runs team meetings and patient case conferences to assure plans of care are aligned with specific team members and outcomes are achieved. Meetings are to be set regularly and have a defined agenda
Qualifications:
Applicants must be a graduate of an accredited school of nursing, therapy or social work Bachelor’s degree in nursing, therapy or social work or has equivalent experience Must possess NH clinical license with NH Board of Nursing Minimum five years’ recent acute care experience, three to five years’ experience in a supervisory role, excellent time management, multi-tasking, verbal and written skills, competent in problem solving Ability to exercise initiative and independent judgement. Management experience required. High proficiency in EMR systems is also required Preferred Qualifications Master’s degree in nursing, therapy or social work Homecare experience Extremely knowledgeable in Medicare Conditions of Participation (in Homecare and Hospice) Community Health/Education experience
Certifications:
