- Direct dailyoperations of the Case Management department: from census andresource management to compliance and discharge processes.
- Supervise, mentor, and develop your team to consistently exceedquality and performance goals.
- Advocate for patients andfamilies, ensuring smooth transitions and personalized,compassionate care.
- Foster strong partnerships with payors,self-insured employers, and external care coordination firms.
- Champion continuous improvement and celebrate wins—big or small.
Job Details
- CCM or ACM certification not required to start, butwant candidate to obtain certification within the first year,hospital will pay for certification
- Need to be licensed if RN,social worker or therapist
- Open to either RN, licensed socialworker or therapist (PT, OT, SLP)
- must have acute care casemanagement, discharge planning, and utilization review experienceplus leadership experience (manager or above)
- Total directreports: 5 total (4 case managers, 1 case manager assistant); 4case managers are 2 RN and 2 social workers
- Hospital ADC is 44;Total beds is 50 and stays pretty full
- Typical case load expectedof the case managers is 11-13
- There are times when DCM takes acase load and has to come in after hours or weekends; DCM doeson-call weekend (Sr.
