Health Coordinator 4.2 4.2 out of 5 stars 207 Locust Street, Dover, NH 03820 Dover Children’s Home is seeking a compassionate, highly skilled Health Coordinator to join our dedicated team. This individual will work with youth ages 10- 21, their providers, and community-based agencies to provide holistic health services and to support youth in healing from trauma. The Health Coordinator will be responsible for supporting youth advocacy, promoting evidence-based agency models, providing health-care coordination, supervision of interns, operational support, and maintaining strong administrative and financial practices. SUPERVISION

Provide weekly formal supervision and evaluations for undergraduate level interns, when applicable. Prepare for and attend weekly supervisions with the Youth Service Manager, as the supervisor for this role.

TREATMENT SERVICES

Treat youth, families, colleagues, stakeholders, and community members with respect, empathy, and dignity at all times. Provide direct oversight and supervision of youth in care while supporting holistic health coordination. Utilize safety plans, provide crisis intervention, and coordinate additional medial and health interventions, as needed. Collaborate with DCH’s coordination team to provide comprehensive and holistic treatment services for all youth in care by developing and implementing individualized treatment plans based on evidence-based practices and a trauma-informed approach and developing a plan for each youth to eat satiating and nutritionally dense food. Advocate for youth’s permanency goals and drive efforts within the youth’s reintegration plan in collaboration with the youth and their team. Schedule and coordinate communication with all medical professionals, attend medical appointments, as necessary, and provide clear and effective communication about the youth’s medical care with the guardians, State workers, and/or additional providers, in collaboration with the youth to honor youth voice and choice. Develop, implement, and engage youth with intentional and ongoing treatment-related groups, aimed to provide education and practice related to health recommendations. • Attend all youth-related meetings, including treatment meetings, RENEW meetings, IEP meetings, etc., as appropriate.

POSITION DESCRIPTION

Health Coordinator Job Description Foster relationships with community-based agencies to build connections and increase services for youth. Assess and support all youth medical and physical health needs during their intake to determine the most appropriate services and provide recommendations to support the continuum of care upon discharges, according to all state timelines. Complete thorough documentation to include electronic records, daily charts, incident reports, monthly reports, treatment goals/objectives, required assessments and/or discharge documentation within the identified timeframes. Ensure that all services and documentation is accurate, up-to-date, and in compliance with accreditation standards, ethical codes, and licensing requirements. Attend Safety Committee meetings and monitor and maintain all medical supplies within the home including over the counter medications, first aid/CPR equipment, and other youth-specific needs to support their health. Uphold the principles of Trust-Based Relational Intervention (TBRI) in all interactions with youth, ensuring fidelity to the model of care. Support Resiliency, Empowerment, Natural supports, Education and Work (R.E.N.E.W.) model with youth, youth’s teams, and in collaboration with DCH’s

team. Safely administer medication for youth as outlined by DCH’s policy and procedure manual and participate as part of DCH’s medication team. Provide up to 6-months of after-care services to discharged youth and coordinate monthly resource night activities for any/all past DCH residents in collaboration with DCH’s aftercare team, as assigned.

OPERATIONAL

Ensure timely and effective communication with all stakeholders (providers, guardians, case workers, etc.) regarding medical changes, progress, and concerns. Maintain electronic records, treatment documentation, and other youth needs in compliance with regulatory standards, confidentiality laws, and Dover Children’s Home policies. Complete all training requirements thoroughly for role, agency, and license (if applicable) within the identified timeframes. Participate in the on-call rotation. Maintain responsibility for the physical safety and cleanliness of the organization, including personal offices/office spaces and shared spaces in the programs. Promptly report any household safety concerns to the Facilities Coordinator and Director Team.

ADMINISTRATIVE

Follow organizational protocols for accepting and documenting donations, ensuring proper communication with relevant team members. Represent the organization with professionalism and gratitude toward donors, emphasizing the importance of their contributions.

FINANCIAL

Make fiscally responsible decisions that align with organizational policies, ensuring proper oversight and minimizing financial burdens on the organization. Adhere to all financial policies and procedures to maintain accountability and transparency. Communicate yearly budgetary needs and financial requests to the Youth Service Manager as requested. Ensure timesheets are up-to-date, accurate, and overtime is managed in a fiscally responsible way.
