Sun River Health is a Federally Qualified,Non-Profit Health Center serving communities in Suffolk, Rockland,Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester. Wecurrently have the need for a full-time Nurse Manager to supportour Inwood Health Center. Position Summary A Nurse Manager rendersprofessional nursing care to patients, within an assigned unit orauthorized community health center site, in support of medical careas directed by the medical staff and pursuant to the objectives andpolicies of the Hudson River Community Health, participates in theinterview process, make hiring decisions, train staff on policiesand procedures, conduct performance reviews, and follow progressivediscipline guidelines as needed and is responsible for conductingmonthly quality health standard audits of assigned units. Performsother duties as assigned on as needed basis. Essential Duties andJob Responsibilities Provide patient education according topatient’s needs and preferences and document in medical recordAttend mandated training and demonstrate competence on all nursingprocedures, including relevant lab procedures Manage/lead theclinical staff (nurses, medical and clinical assistants, laboratorystaff, and, and Patient Navigators), including but not limited torecruitment and hiring, performance reviews, onboarding, trainingand competencies, assignments, and workflow Ensure that allclinical staff (nurses, medical and clinical assistants, laboratorystaff, and, and Patient Navigators) adhere to and understand theworkflows, documentation requirements in EHR according to HRHCAREquality standards Review all site quality data monthly andidentify, lead and develop strategies for improvement incollaboration with the site leadership team, to promote quality,safety and patient satisfaction Ensure that site champions areidentified for the core quality initiatives and core systemsincluding workflow and documentation are understood and executed.
Ensure that HRHCare standard procedures and workflows areimplemented and followed, including but not limited to: medicationmanagement, pre-visit planning, team huddles, accurate and timelycompletion of Nursing Quality Logs, exam room readiness, orderingand storing of supplies and equipment Responsible for oversight oflaboratory operations and continuous review of In-House Lab TestsQuality Control and Maintenance Logs Ensure that the quality ofcare is assessed and improved through regular record audits Incollaboration with the rest of the site leadership team, developand lead initiatives to promote and improve quality, safety andpatient satisfaction Participate in monthly leadership and staffmeetings Demonstrate knowledge of decontamination and sterilizationprocesses, and perform regular review of D & S Processing andMaintenance Logs (note: in sites with dental services, dentistperforms more intensive review of logs, but NM needs to be familiarwith and shares responsibility for compliance with standards)Conduct monthly component meetings with all clinical support teamand review the priority issues of the organization includingquality, safety and patient satisfaction including anyorganizational updates or initiatives Complete annual evaluationsof clinical support team Monitor and facilitate patient follow-upas needed, including test results and missed appointments.
Demonstrate knowledge of the principles of growth and developmentover the life span, and provide age- appropriate and culturallysensitive care, including use of medical interpreters as indicatedPerform direct patient care duties of an
RN:
interview patients forhealth history, prepare equipment and assist practitioner duringexamination and treatment of patients, document in patient’smedical record according to HRHCare standard, assess and triagepatients as needed
