The Patient Care Coordinator is responsible for thecompletion of set processes and protocols. Works cooperatively withall members of the care team to support the vision and mission ofthe organization, deliver excellent customer service and adhere toLean processes. Supports the teams in meeting financial, clinicaland service goals.

Working Hours:

A 40-hour work week Mondaythrough Thursday between the hours of 8:00 am to 4:30 pm, Fridaybetween the hours of 8:00 am to 2:00 pm. Schedule to be determinedby manager upon hire. May be asked to float to other practices asneeded.

Primary Responsibilities:
  • Obtain accurate and updated patientinformation, such as name, address, insurance information
  • Performinsurance verification on the date of service
  • Obtain patientsignatures for required documents
  • Upload a valid Governmentissued state ID of patient and patient insurance cards
  • Accurateand complete occupational medicine registration workflow
  • File andmaintain medical records
  • Consistently correct registration workqueue errors
  • Confirms and schedule appointments
  • Answertelephone calls promptly and courteously
  • Perform referraldocumentation promptly
  • Working daily in the claims edit workqueue to correct registration errors for submission to insurancecompanies
  • Performs certain follow-up services for patients in aprompt and courteous manner, such as scheduling specialistappointments, scheduling follow-up appointments and obtainingcopies lab results or specialist reports
  • Adhere to the standardsidentified via Sparq regarding Optum Employee Policies
  • Workcohesively with fellow employees to achieve specific team goals
  • Keep abreast on current medical requirements relevant to theirposition, which includes maintaining patient confidentiality andabiding by all HIPAA and OSHA requirements
  • Comfortable working inhigh pace environment
  • Participate in planning, coordinating, andimplementing care in conjunction with the multidisciplinary team inaccordance with the Plan of Care
  • Assure the continuity of carethrough scheduling and tracking systems
  • Provide effectivecommunication to patient / family team members and other healthcare professionals as evidenced by documentation, case conferences,communication notes, and evaluations
  • Notify clinical staff ofschedule changes as they occur
  • Comply with administrativepolicies to ensure quality of care
  • Demonstrate precision andefficiency in scanning documents and monitoring the fax server,retrieving and / or scanning documents and assigning to theappropriate electronic chart
  • Participate in orientation(scheduling segment) of new field employees and distribute updatesand changes as needed
  • Perform other related duties andassignments as required
  • Organize and maintains medical records

Required Qualifications:
  • High SchoolDiploma/GED (or higher)
  • 1 year of experience working in medicaloffice position performing duties such as scheduling appointments,checking patients in/out, insurance verification, collectingco-pays and maintaining medical records
  • Intermediate level ofcomputer proficiency including MS Word, Excel, Outlook and theability to use multiple web applications
  • Ability to travel 10% ofthe time to cover other offices as needed, and have reliabletransportation
Preferred Qualifications:
  • Knowledge of Epic EMRsoftware
  • Bilingual in

  • Abilityto work independently and maintain good judgment and accountability
  • Demonstrated ability to work well with health care providers
  • Strong organizational and time management skills
  • Ability tomulti-task and prioritize tasks to meet all deadlines

  • Excellentverbal and written communication skills; ability to speak clearlyand concisely, conveying information in a manner that others canunderstand, as well as ability to understand and interpretinformation from others Pay is based on several factors includingbut not limited to local labor markets, education, work experience,certifications, etc.
