Description CSPNJ, a mental health nonprofit agency, is looking for a full-time Clinical Plan Coordinator (CPC). Under the direction of the Regional Coordinator, the CPC will assess individual client needs and plan services to be delivered by line staff. The CPC will clinically supervise line staff by providing CSS support services to individuals with mental health diagnosis while living in a supportive housing environment. Responsibilities include conducting Comprehensive Rehabilitation Needs Assessments (CRNAs) and Individual Rehabilitation Plans (IRPs).
Highlights:
45% to 50% of time spent in direct face to face billable contact with consumers.
45% to 50% of time should be spent on training, clinical supervision, note review, and administration of the team and region.
Performs comprehensive rehabilitation needs assessments and individual rehabilitation plans, scheduling them in advance to ensure timely completion of each.
Maintains accurate records related to the Individual Rehabilitation Plan and support services provided by the team. Follows & enforces team protocol for charting and recording current treatment information for residents.

REQUIRED
Master’s Degree in a mental health related discipline.
REQUIRED
NJ Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN).
1+ years of experience in a supervisory role.
Ability to work effectively under pressure and manage crises.
Time and project management skills.
Ability to analyze processes and information, identify problems and trends, and develop effective solutions and strategies.
Excellent communication, interpersonal, leadership, coaching, and conflict resolution skills. Foster teamwork.
Capable of making sound conclusions and excellent judgment in clinical situations.
Ability to work with individuals with mental illness in a caring and professional manner.
Proficient in MS Office products, as well as general computer literacy.
Bilingual skills are a plus.
Valid NJ Driver’s License with acceptable driving record.

Annual Salary Range:
$70,000.00 – $77,500.00
