  • Coordinates and facilitates the care ofthe patient population through effective collaboration andcommunication with the Interdisciplinary Care Transitions (ICT)team members
  • Follows patients throughout the continuum of careand ensures optimum utilization of resources, service delivery andcompliance with external review agencies
  • Provides ongoing supportand expertise through comprehensive assessment, care planning, planimplementation and overall evaluation of individual patient needs
  • Enhances the quality of patient management and satisfaction, topromote continuity of care and cost effectiveness through theintegration of functions of case management, utilization review andmanagement, and discharge planning Essential Functions CareCoordination
  • Coordinates clinical and/or psycho-social activitieswith the Interdisciplinary Team and Physicians
  • Monitors all areasof patients’ stay for effective care coordination and efficientcare facilitation
  • Remains current from a knowledge baseperspective regarding reimbursement modalities, communityresources, case management, psychosocial and legal issues thataffect patients and providers of care
  • Appropriately refers highrisk patients who would benefit from additional support
  • Serves asa patient advocate. Enhances a collaborative relationship tomaximize the patient’s and family’s ability to make informeddecisions
  • Demonstrates knowledge of the principles of growth anddevelopment over the life span and the skills necessary to provideage appropriate care to the patient population served
  • Participates in interdisciplinary patient care rounds and/orconferences to review treatment goals, optimize resourceutilization, provide family education and identified post hospitalneeds Collaborates with clinical staff in the development andexecution of the plan of care, and achievement of goals
  • Coordinates with interdisciplinary care team, physicians, patients,families, post-acute providers, payors, and others in the planningof the patients’ care throughout the care continuumKnowledge/Skills/Abilities/Expectations
  • Knowledge of governmentand non-government payor practices, regulations, standards andreimbursement
  • Knowledge of Medicare benefits and insuranceprocesses and contracts
  • Knowledge of accreditation standards andcompliance requirements
  • Ability to demonstrate critical thinking,appropriate prioritization and time management skills
  • Basiccomputer skills with working knowledge of Microsoft Office,word-processing and spreadsheet software
  • Excellent interpersonal,verbal and written skills in order to communicate effectively andto obtain cooperation/collaboration from hospital leadership, aswell as physicians, payors and other external customers
  • Demonstrates good interpersonal skills when working or interactingwith patients, their families and other staff members
  • Approximatepercent of time required to travel, 0%
  • Must read, write and speakfluent English
  • Must have good and regular attendance
  • Performsother related duties as assigned
Salary Range:

$35.22

  • $50.

Qualifications Education

  • Graduate of an accredited programrequired for RN. BSN preferred; or MSW/BSW with licensure asrequired by state regulations Licenses/Certification
  • Healthcareprofessional licensure required as Registered Nurse, or LicensedClinical Social Worker (LCSW) or Licensed Social Worker (LSW) ifrequired by state regulations
  • Certification in Case Management aplus Experience
  • Two years of experience in a healthcare settingpreferred
  • Prefer prior experience in case management, utilizationreview, or discharge planning
